We are committed to continuously seek opportunities to learn and improve processes to prevent defects, harm and create a culture of safety in our organization. In honor of celebrating your commitment, the System Quality and Patient Safety nomination committee is inviting nominations for the 2022 Culture of Safety Awards.
Purpose: Recognize outstanding individuals and teams’ consistent contributions and commitment in advancing quality and patient safety in their units, departments, hospitals and/or the system.
Eligibility: Physicians and all employees, both clinical and non-clinical, may nominate an individual or a team (maximum of 10 individuals). Private physicians who do not have access to MARS and wish to nominate may contact Pavithra Bora.
Process: Nominations must be submitted using the BRAVO platform only. Nominations must be in alignment with the award category descriptions. Use this job aid to help with your nomination. Please note BRAVO times out every 20 minutes.
Deadline: Nominations are due by Aug. 10 at 5 p.m. Awardees will be notified on Oct 18. For more information, contact Pavithra Bora.
